Once your first appointment is scheduled, our office will e-mail you a new patient information packet. This will include information about the practice as well as a patient registration form that needs to be filled out and brought with you to your first appointment.
Patient Registration Forms
A Picture ID (Military ID or Drivers License)
Bring a bag containing all current medications, over-the-counter medications, and
(A complete list if you cannot bring the items with you.)
Any lab work completed within the past year, or x-rays/immaging completed in the past 5 years pertaining to the reason for your visit.
Payment for your specialist office visit co-pay. We accept checks, cash, Master Card, and Visa.
If you do not know how much your co-pay will be, feel free to call us ahead of time and we can look it up for you.
Wearing loose comfortable clothing to your appointment is ideal.
Specifically, we recommend gym clothes or a t-shirt and sweat pants.
Drink lots of water!
Our goal is to create a calm and tranquil environment. We do ask that all cell phones be turned off once you are in the office. Because many of our patients have respiratory problems as well as sensitivities to chemicals, we ask our patients to refrain from wearing strong fragrances or perfumes. This includes not smoking prior to your visit or wearing clothing that strongly smells of smoke. Due to the nature of OMT, you may want to arrange for childcare during your appointment, as your treatment will be more effective if you are able to relax and are not continually distracted with the duties of parenting. We do welcome family and friends of our patients, but ask that they too abide by these requests.
At Old Dominion Osteopathic Medicine, we do our best to be respectful of our patient’s time. We do not double book or over schedule. As a result, it is important that our patients arrive on time. If you arrive late for a scheduled appointment, your physician may not have sufficient time to adequately care for your needs, but we will be happy to schedule you for another appointment time.
We ask that if your appointment needs to be cancelled or rescheduled that this is done no less than twenty-four hours in advance. This will allow us to offer the appointment to another patient and will prevent you from being charged for the scheduled time.
